Guidelines for Requesting Amendments to Outcome Reports
At Care ADHD, we take great pride in providing high-quality, evidence-based assessments for adult ADHD. The outcome report following your assessment is an important document that reflects the clinician’s findings and reasoning behind the diagnosis. We understand that, on occasion, you may wish to request amendments to the report. These guidelines outline when such requests are appropriate, and we encourage you to review them carefully to understand the process, and what we may or may not be able to amend.
Our aim is to ensure that your report is as accurate and complete as possible.
Acceptable Amendment Requests
We understand that you may have questions or wish to clarify specific details within your report. We are happy to consider amendment requests in the following limited circumstances:
Factual Errors or Omissions:
If there are any factual inaccuracies or omissions, such as incorrect personal details (e.g. name, date of birth) or misreported test results, we will correct these promptly.
Significant Information Missing:
If a key piece of information that directly influences the clinical understanding of your case is missing, we will update the report to reflect this. For example: A relevant aspect of your medical history or previous treatments that was not included but was part of the assessment.
Requests We Cannot Accept
While we are committed to delivering a comprehensive and accurate report, there are certain types of amendment requests that we are unable to process. These are in place to ensure the integrity of the report and the accuracy of the diagnosis:
Important Note on Diagnostic Conclusions
The diagnosis and clinical findings presented in the outcome report are the result of a comprehensive, evidence-based assessment. This includes information gathered through standardised rating scales and questionnaires completed prior to the clinical interview, as well as the clinical interview itself.
Please note that we are unable to revise the diagnosis or clinical conclusions based on new information provided after the assessment, unless such information was discussed during the assessment process. Any consideration of additional evidence or clarification will be at the discretion of the assessing clinician.
If you do not agree with the diagnosis provided, you are entitled to seek a second opinion. However, as noted above, we are unable to revise or challenge the original diagnosis based on new information submitted after the assessment.
Should you wish to request a second opinion within our service, this would be conducted by an alternative assessor. Please note that second opinions would be arranged privately and would incur an additional cost.
Requests to Remove References to Drug Use or Illegal Activity:
If your report includes references to drug use or illegal activity that were discussed during the assessment and are relevant to your clinical evaluation, we are unable to remove or alter these references. These are important for the overall clinical picture and cannot be omitted.
Example: A mention of past recreational drug use or legal matters that are relevant to the assessment of your condition.
Changes to the Tone or Interpretation:
The report reflects the clinician’s professional assessment of the behaviours and symptoms discussed during the evaluation. While we are happy to clarify any points that may seem unclear, we cannot amend subjective interpretations or change the tone to better align with personal preferences.
Example: A request to reword the clinician’s interpretation of specific symptoms or behaviours in a more positive light.
Removal of Clinically Relevant Information:
If the information included in the report is deemed clinically relevant to your diagnosis, it will not be removed or altered unless there is a factual error. We cannot omit significant details that contribute to understanding your condition.
Example: Removing references to aspects of your medical or personal history that are crucial to the clinical evaluation.
Amendment Request Process
To ensure clarity and efficiency, we ask that all amendment requests follow a clear process:
One Request for Amendments:
We can accept one amendment request per report.
This request must be made in writing, as a response to the original outcome letter email (please do not send your email to our general enquiries email address).
Your request must be sent to us within 5 working days of your final report being sent to you.
Please ensure that your request clearly specifies the section(s) you would like amended, the change(s) you would like to be made, and the reason(s) for the request.
Review by Clinician:
Once received, your amendment request will be reviewed to ensure it meets the criteria of acceptable requests as outlined above. If it does, your request will be shared with your clinician, who will review the details and make any agreed amendments, if appropriate. You will be informed of the decision and, where applicable, sent an updated version of the report.
Please note that we are unable to guarantee that every amendment request will be actioned. Amendments are at the discretion of the assessing clinician but we will endeavour to accept all verified requests that meet the eligibility criteria outlined above.
Final Report:
After the requested amendments have been made, the updated report will be sent to you and your GP. Once the amendments are finalised, no further changes can be made to the report.
We are here to provide the highest standard of care and to ensure that your assessment report accurately reflects the findings of your evaluation. While we do allow for amendments in certain circumstances, it is important that all requests are made within the scope outlined above. Our goal is to provide you and your GP with a comprehensive, accurate, and clear report, and these guidelines help to ensure transparency on the type of amendment requests we can facilitate.
