Medication Monitoring During Titration

Before each scheduled medication review, you’ll need to complete a treatment monitoring form. This form helps our clinicians assess how well your medication is working and identify any concerns or side effects you may be experiencing. The form is hosted on our website and can be easily accessed and completed online via this link. 

The form provides crucial information that allows the clinician to: 

• Review how your medication is affecting you. 

• Address any concerns or side effects you may have experienced. 

• Issue a new prescription, based on your progress. 

You should complete the treatment monitoring form on our website before each titration/medication review appointment. This allows us to prepare for your appointment and make the most of your time with the clinician. 

After reviewing your form and discussing your progress, the clinician will: 

• Adjust your medication dosage or type if needed. 

• Issue a prescription to continue your treatment. 

• Offer guidance on managing any side effects or concerns. 

• Complete the Form Ahead of Your Appointment: Make sure to submit your monitoring form before each review to help ensure a smooth consultation. 

• Be Honest About Side Effects: If you’ve noticed any side effects or difficulties with your medication, it’s important to share this information with us. 

• Continuous Monitoring: The titration process involves regular reviews to adjust your medication as needed, ensuring your treatment remains effective and safe. 

If you feel unwell at any point due to your medication, or you require urgent medical assistance: 

• ❌ Stop taking the medication immediately. 

• ✅ Seek help from your GP or visit your nearest hospital. 

ADHD medication can be discontinued abruptly without adverse effects and does not require tapering off, unlike some other medications.
