What to expect during titration, and what’s included in the service

Titration is a structured 12-week pathway designed to help you find the most effective and safe ADHD medication and dosage for your needs. During this process, our clinicians will closely monitor your progress, adjust your treatment as necessary, and support your transition to stable, long-term care. 

Please be aware that Private Patients have T1, T2 and T3 included in their titration package. Additional titration appointments can be purchased as necessary

Overview of the pathway

Before you enter the titration pathway, our clinicians will perform a clinical review of your medical history to make sure it’s clinically safe for you to commence treatment. If further investigations are needed (for example, an ECG or repeat blood pressure readings), we’ll be in touch to discuss this.

When you’re ready to commence treatment, you'll be invited to complete some forms in our patient portal. We'll be notified automatically once you've completed all the forms, and soon after our Bookings team will be in touch to schedule all of your titration appointments, so you'll know exactly when you’ll be meeting with your clinician throughout the pathway journey.

1. Titration appointments

You will begin the pathway with your first appointment (“T1”) where you'll discuss the medications we can offer, your treatment goals, and the next steps.

Following your appointment, your clinician will send your prescription to our partner pharmacy. The pharmacy will contact you directly via email (usually within 3 working days) with a link to complete your payment for the medication. Once payment is made, the pharmacy will dispatch your medication directly to your home address. If payment is processed promptly, medication typically arrives within a week of each titration appointment.

After your first titration appointment, you’ll have two further appointments (“T2” and “T3”) scheduled during weeks 4 and 8 of the pathway. NHS RTC patients will also be offered an additional appointment T4 by default. This schedule allows for medication reviews and ensures enough time for your prescription to be processed and the next batch of medication to arrive before your current supply runs out.

To avoid delays, it is important to process payments promptly for each batch of medication. However, if your next batch of medication does not arrive before your current supply runs out, you can reduce or stop taking your ADHD medication while awaiting your next batch. ADHD medication can be discontinued abruptly without adverse effects and does not require tapering off, unlike some other medications.

If you need to adjust the date of your pre-scheduled appointments, we recommend bringing the appointments forward to prevent disruptions to your medication supply. Pushing an appointment back may result in running out of medication before your next batch arrives.

Please note that at least 2 working days' notice is required to reschedule an appointment without incurring additional fees. If less than 2 working days' notice is provided, or if you fail to attend your scheduled appointment, there will be a fee of £75 to reschedule your appointment.

2. Shared Care Agreement

Once your medication is stabilised (typically after 8 -12 weeks or more), we can send a shared care agreement to your GP, if you choose. This allows your GP to take over prescribing and ongoing management of your medication.

If Shared Care is agreed, you’ll pay the NHS rate for your medication, which will be prescribed by your GP (currently £9.90/item, or free for those who are eligible). As part of the Shared Care agreement, you’ll have an annual review with us each year, which is currently priced at £140 for private clients, or free for NHS Patients on the Right to Choose pathway.

3. Ongoing prescriptions

If your GP rejects shared care or you wish to continue buying your medication privately, we can supply the prescription for you at a cost of £20/month. This charge is for the processing of your prescription and does not include the cost of your medication, which is paid directly to the pharmacy.


Additional options and support

⏰ Extended Titration

If your medication is not stabilised after the 12 week pathway and you require further adjustments, or you wish to schedule an additional medication review at any point during or following titration, additional appointments are available. For private patients, the additional 4th appointment (ad hoc appointments) are at an additional cost.

Right to Choose patients have access to up to 9 titration appointments during the titration pathway, but further appointments after this period are available.

🚨 Emergency Support

Please note that emergency support is not included in our titration service. If you feel unwell due to your medication or require urgent medical assistance:

  • Stop taking the medication immediately.

  • Visit your nearest hospital or GP for urgent care.

Please also discuss any adverse reactions or medical concerns you may have with your clinician at your next appointment.
